Pneumonia is an inflammation of the lung parenchyma, often confined to a single lobe or section of a lobe. Bacterial, viral, or fungal infection may be the cause. Pneumonia is a leading cause of death in the U.S. (and worldwide), though overall mortality is low and most fatalities occur in infants, the elderly, hospitalized patients, and the immunocompromised. It typically begins with flu-like symptoms which develop into fever, productive cough, painful breathing, and dyspnea, possibly to the extent of hypoxia and cyanosis. Numerous causative pathogens have been identified and can often be predicted based on patient characteristics, such as age and immune status, as well as whether the infection was community- or hospital-acquired. Though severe cases require hospitalization, mild cases can be treated on an outpatient basis; the CURB-65 tool can be used to assess severity and help determine whether hospitalization is warranted.
