Multiple sclerosis is an autoimmune neurological disease, which causes incoordination and loss of function through the progressive degradation of myelin sheaths throughout the central nervous system. Although it was not clearly categorized as a distinct disease until the 19th century, when the French neurologist Charcot discovered its pathological basis, descriptions of symptoms closely resembling multiple sclerosis date at least as far back as the Middle Ages, meaning the disease is likely far older than many of the modern toxic exposures often accused of being its cause. The cause remains unknown, but many intriguing etiological theories have been put forth, including dairy consumption, chronic viral infection, pesticide exposure, heavy metal exposure, mercury amalgam fillings, the polio vaccine, and vitamin D deficiency. However, none of these theories have provided sufficient evidence to be widely accepted. What is known is that multiple sclerosis has a predilection to most commonly afflict young women of northern European descent and that depression is a common comorbidity. Even Charcot, who named the disease after discovering plaques in the central nervous system responsible for its debilitating symptoms, had theorized a possible emotional origin, and antecedent psychosocial stress is now an important acknowledged risk factor. Augustus D’Este, the grandson of England’s King George III, famously suffered from multiple sclerosis and kept a diary, in exquisite detail, recounting his lifelong battle with the disease. He blamed the visual symptoms which heralded the onset of his disease to irritation caused by tears he had forcibly withheld while attending a friend’s funeral. The presenting symptoms are variable but classically include paresthesias, visual disturbances, increasing weakness, stiffness, or clumsiness noted in one or more limbs, vertigo, and urinary incontinence. The onset is insidious, and in the earliest stages, the symptoms may be confused with a conversion disorder or even dismissed entirely. The disease generally follows a relapsing-remitting course, though a more severe, relentlessly progressive course is also encountered. Spontaneous remission has also occurred in some cases. Though it often has no impact on actual life span, the progressive debility can severely affect patients’ quality of life.
